General Liability Waiver I, , hereby acknowledge that participation in any physical activity at Gold Coast Gymnastics ("GCG") located at 4005 E Main Street, Ventura, CA involves inherent risks, including but not limited to slips, falls, sprains, fractures, and other injuries. I voluntarily assume all risks, known and unknown, associated with participation in classes, open gyms, private lessons, events, or use of the facility.

I affirm that {name} is in good health and physically able to participate in gymnastics and related activities. If {name} is a minor, I confirm that I am the parent or legal guardian and have full authority to sign this waiver on their behalf.

I understand and accept the risks involved.

Indemnification I agree to indemnify, defend, and hold harmless Gold Coast Gymnastics, its owners, coaches, staff, and volunteers from any and all claims, damages, losses, or expenses arising out of or resulting from participation in any GCG activity, including claims of negligence.

I agree to the indemnification terms.

Medical Release In the event of an emergency, I authorize the staff of Gold Coast Gymnastics to take all reasonable steps to secure emergency medical treatment for {name} as deemed necessary. I understand that I am responsible for all medical expenses incurred.

I agree to the medical release.

Photo and Media Release I give permission for Gold Coast Gymnastics to photograph or record {name} while participating in classes or events. I authorize GCG to use these images and videos for marketing purposes, including but not limited to social media, print materials, and the website.

I agree to the Photo/Video Release. I DO NOT agree to the Photo/Video Release.

Acknowledgment of Gym Policies I agree to follow all gym rules and safety protocols. I understand that failure to follow staff directions or gym policies may result in removal from the program or facility without refund.

I understand and agree to comply with gym policies.

Drop-In / Open Gym / Private Use Disclaimer I understand that participation in open gym, private lessons, or rentals may not be directly supervised at all times and I assume full responsibility for my actions or my child's actions during these times.

I agree to the open gym and private use terms.

Severability If any provision of this waiver is found to be unenforceable or invalid, the remaining provisions shall continue in full force and effect.

Acknowledgment and Signature By signing below, I confirm that I have read, understood, and voluntarily agree to all terms listed in this waiver. I acknowledge that I am signing this freely and without duress. This waiver will remain in effect for all future participation at Gold Coast Gymnastics unless revoked in writing.
